Description: Iowa City Hospice is seeking a Hospice Pool Nurse to be a part of our interdisciplinary team.
Iowa City Hospice Nurses have a passionate understanding for the hospice philosophy.
They combine their acute/crisis symptom management education with interpersonal communication skills to provide care for patients and families in physical and emotional crisis situations.

Our Hospice Nurses: Assess home care and physical needs of the patient, regardless of the home location.
Identify and coordinate patient/family care with other members of the Interdisciplinary Team and health care community as needed.
Communicate with the primary physician regarding patient change in status and pain and symptom control.
Teach and counsel patients/families on care giving techniques.
Facilitate admission of hospice patient/family when necessary.
Document observations, treatments, and other pertinent information into an IPad electronic medical records system.
Schedule: Flexible; based on your availability.
EMPLOYMENT STATUS: Part Time Hourly, non-exempt REPORTS TO: Hospice Nurse Manager Essential Job Functions (including but not limited to): To be performed satisfactorily with or without reasonable accommodation.

1.
Complete the initial and ongoing patient assessment and ensure that orders for treatment and services are obtained to meet the needs of the patient.
2.
Provide care that is in accordance with accepted nursing standards of practice and agency policies and procedures.
3.
Ensure electronic medical record accurately reflects visit schedule per agency expectations.
4.
Attend morning huddle with Team Coordinator via phone at 8:10 AM 5.
Ensure the patient’s medication list is accurate through reconciliation with the facility or home medication lists every visit.
6.
Ensure nursing care and services provided are in accordance with the patient’s plan of care and goals are based on the patient’s identified needs for the palliation and management of symptoms.
7.
Diffuse conflict with respectful, calm collaboration 8.
Maintain open, approachable manner.
9.
Treat others fairly and respectfully.
10.
Supervise care delegated to a Licensed Practical Nurse and nurse aide(s).
11.
Communicate with the primary physician and team coordinator per agency policies and processes 12.
Document all care provided on the patient’s clinical record in accordance with agency documentation requirements 13.
Participate in facility patient care conferences via telehealth, or in person, in coordination with interdisciplinary team 14.
Gather data to assist in re certification process.
15.
Demonstrate exceptional written, electronic and verbal communication standards.
16.
Establish and maintain strong relationships with the agency’s care partners.
17.
Train patients, care partners and caregiver about symptom management, the safe administration of drugs and care giving techniques.
18.
Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
19.
Participate in Iowa Care Community’s success by ensuring access to our exceptional care.

Support and promote Iowa City Hospice’s mission 21.
Provide excellent interdisciplinary patient care through collaboration, communication, and coordination with the interdisciplinary team, team coordinator and facility care partners.

Perform other duties as assigned Requirements: Licensure: Current Iowa Registered Nursing License Education: Associate degree with relevant experience Experience: Two years of hospice, long term care, assisted living or hospital related work experience preferred Driving: Proof of license and current insurance coverage Criminal background check: All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law and a negative finding on the Office of Inspector General’s Excluded Provider List.
