Shelter House's Permanent Supportive Case managers are skilled communicators who help our most vulnerable tenants maintain housing, secure health care and other benefits, and offer a patient and pragmatic ear.
Description: The Permanent Supportive Housing Case Manager is an experienced, human services professional responsible for providing high-quality, comprehensive, and intensive case management services to people formerly experiencing homelessness.
The role of the Permanent Supportive Housing Case Manager is to build trusting relationships with program participants which will enable the participants to maintain housing and reach personalized goals.
The Case Manager is dedicated to supporting and empowering participants utilizing evidence-based practices, including a Housing First philosophy, Motivational Interviewing, Harm Reduction, and Trauma Informed Care.
Key Responsibilities:
1.
Provide case management services to formally homeless clients so that clients maintain housing, and client’s health, safety, and welfare needs are met.
2.
Build rapport and maintain positive professional relationships with clients, landlords, service providers, and other community partners.
3.
Accurately record and maintain client data and statistics including but not limited to demographics, outcome measures, goals, face-to-face notes, medical information, collateral contact, and grant-specific requirements, and maintain all client records to ensure accuracy, confidentiality, and security in a timely manner.
4.
Work within a team of Case Managers and Coordinators to ensure continuity of care, this includes attending team meetings, agency-wide meetings/trainings, and frequent communication with the Iowa City Housing Authority.

Requirements:
Knowledge, skills and abilities consistent with Bachelor’s degree in in human services related field, or minimum one years related experience.
Demonstrated knowledge and experience with area services, community partners, and resources.
Commitment to operating within a trauma-informed workplace that demonstrates a balance of kindness, competence and care toward self, colleagues, and populations served.
Minimum of one-year direct service experience.
Proficiency in Windows based computer environment and Microsoft Office Suite.
Good written, verbal, and interpersonal communication skills.
Possess a valid driver's license and reliable transportation to get to and from work shifts.
